SHRAC--Statewide Human Rights Advocacy Committee

SSI--Supplemental Security Income

Support Coordination--Process in which supports and services are developed to meet individual goals. Previously known as "case management," its' focus is the development of a persons capacity to negotiate and obtain services on their own behalf.

Support Coordinator--A person who assists individuals and families in identifying their needs and resources and locates and/or gains access to necessary supports and services.

Supported Employment--The individual employment, within a regular business or industry, of a person with disabilities, made possible by (a) careful matching of worker/job characteristics, (b) intensive advocacy and on-site training in work and related skills by a job coach, and (c) subsequent ongoing support services.

Supported Living--The opportunity to choose where, how, and with whom a person lives. In supported living, a person receives the individualized supports and services needed to live in their own home and community.
